Instructions:
To submit your claim online, you will need the following information to verify your identity: Notice ID (located on the Notice you received). Your 9-digit PIN (located on the Notice you received). Your online claim must be submitted on or before: 11:59 PM (Eastern) on October 12, 2021 Once completed, a copy of your online claim form will be emailed to the address you provide. You should NOT submit both an online claim AND a paper form. Only one claim form will be accepted per qualified individual. The claims period has concluded.
